Service & Event Booking Form
Use this form to book Individual Service, Professional Makeup Artistry, Pamper Parties, Face Painting Parties, Classes/Workshops/Seminars, or Public Speaking/Lecture.
Completion of this form will help in the planning of your service or event. If you don't have all the information, that's ok, provide what you know. We will contact you to discuss and confirm your service or event.
Booking Instructions: A deposit of 50% is required to secure the event date by Cash, Cashier's Check, Credit/Debit Card through Paypal/Google Checkout, or Money Order at least 4 weeks prior to the event. 25% of ths deposit is NON-REFUNDABLE. Deposit is based on total amount of services. We do not accept personal checks for deposits nor will we accept them no less than 14 days before the event/service unless prior arrangements have been made. Please inquire about individual services as they may not require a deposit.
Balance will become due in full at least 7-14 days before event date
- Please take time to complete the booking form in full with all the details required*. This will enable us to plan your schedule and make the event run smoothly.
- List names of guests and services required. For under 18's please state age.
- If Celebrating a special occasions let us know the special occasion and name of bride, groom, birthday boy or girl etc
- There are service/guest minimums to receive specialized pricing. Please refer to the appropriate service page for information
- Depending on location, a travel fee of no less thatn $15 may be applied.
- Booking fee of $15 is applied to services totaling $85 or less.
Relaxing Minds/Spaah2U Bookings Terms and Conditions
1. Relaxing Minds/Spaah2U will make every effort to ensure that each pamper party booking is completed in its entirety
2. In the event that a Service Provider cannot attend a booking as previously agreed, Relaxing Minds/Spaah2U will make all reasonable efforts to find a replacement service provider.
3. Relaxing Minds/Spaah2U shall not be liable to the client for any loss, injury, damage, expense or delay suffered by the Client or the service provider. This exclusion shall extend (but not be limited) to the consequences of any act or omission (whether willful, negligent, reckless, dishonest or otherwise) which takes place during the event on the part of the service provider or the client. The client.
Service Providers
1. Service Providers will act in a professional and respectable manner at all times. Service Providers are self-sufficient, sometimes providing their own equipment for the treatments offered.
2. Service Providers are not employees of Relaxing Minds/Spaah2U. Each is self-employed.
3. The Freelance Service Provider is entitled to fully paid pre-scheduled breaks during the course of the event, these will be detailed on the booking schedule.
4. Freelance Service Providers may refuse treatments to any client deemed to be behaving inappropriately or under the influence of drugs or alcohol, with no refund.
5. Freelance Service Providers are not responsible for any allergic reactions if consent has been given for the respective treatment or allergies were not declared.
6. Freelance Service Providers have the right to charge for any delays caused by the host or guests, resulting in a delay of more than 30 Minutes from the agreed treatment schedule.
The Client
1. The Client may be asked to provide standard tables and chairs.
2. The Client will ensure that adequate room is provided as advised by Relaxing Minds/Spaah2U in order for the Freelance Service Provider to perform the treatments requested and will ensure that the Freelance Service Provider is made to feel welcome upon arrival.
3. Freelance Service Providers require a smoke free environment to provide maximum benefits, and ensure that no products become contaminated.
4. The Client will ensure that all guests have signed appropriate consent documentation prior to receiving treatments at the request of Relaxing Minds/Spaah2U
5. The Client will take all reasonable endeavours to ensure that guests limit the intake of alcoholic beverages prior to receiving Treatments. The legal limit for driving is a suitable guideline.
6. The Client will ensure that guests arrive in time for their pre-scheduled treatments. The Freelance Service Providers will make every effort to accommodate any changes due to lateness but in such an event Relaxing Minds/Spaah2U will have no liability for any loss of treatment time.
7. The Client will ensure that pets and small children will be kept away from all products and equipment to prevent accident or injury.
8. The Client will pay a Non-refundable deposit upon booking the Pamper Party, and the full balance will be due 2 weeks prior to the party date. Once the balance has been paid, this is also non-refundable.
9. Changes made to the party schedule, treatments or address with less than 7 days notice may incur an admin fee. If you decrease the number of guest, the cost of services may increase. You will be required to pay the new service rate.
10. Changes made to the schedule, treatments or address with less than 48 hours notice may result in the termination of the party with no refund. Relaxing Minds/Spaah2U will try to accommodate changes with less than 48 hours notice, however the client may incur a fee of 10% of the balance payment for schedule, treatment and address changes or 20% of the balance payment for address changes more than 4 Miles from the original party location.
Fees
1. All fees will be pre-paid to Relaxing Minds/Spaah2U before the pamper party unless prior alternative arrangements have been made with a representative of Relaxing Minds/Spaah2U
